When our current plane was Canadian registered they used Blackstone, which was nice as Blackstone is our historically preferred vendor, and thus we had some continuity to look at.

+1 Blackstone. Sign up online and they will send you everything you need to send a sample back to them. Everytime you send in a sample, they automatically send you a new sample kit. Results are online and emailed. It's all very convenient.
